Positions are available in health clinics, hospitals, and in private laboratories. The types of patients the professional will work with depends on the facility. However, many of the basic job duties are the same regardless of where one chooses to work.</p><p>In many instances the technician will be required to correctly identify the patient, collect blood specimens, process the blood according to laboratory requirements, and ensure overall quality of one&#8217;s work. It helps to be good at alleviating patients&#8217; fears as many people are fearful of the blood drawing procedure.</p><p>In the hospital setting a person who draws blood may be required to work different shifts or to be available for on-call last minute assignments. In the hospital the professional may work with the elderly, patients in critical care, emergency room visitors, pregnant mothers, and even infants.</p><p>In the private laboratory the professional will work mostly during daytime hours when the lab or facility is open. Some labs will have extended evening hours one or two days per week. Working overtime in a laboratory is rare. Some labs also use traveling phlebotomists who travel to patients&#8217; homes or skilled nursing facilities to collect the specimens. The laboratory may provide a company car or give the professional a car allowance and fuel reimbursement every month.</p><p>In an outpatient clinic the technician may work with people of all ages or with a specific type of patients, depending on the specialty. It is therefore extremely important that you thoroughly understand what your employee insurance will and will not cover.</p><p>Negligence is the most common cause for a patient suing a phlebotomy technician. In the case of personal negligence not only could you be liable for the settlement/award to the plaintiff, you may also be liable for the damages that your employer paid to a claimant. With professional liability insurance, these expenses can be transferred to your personal insurance. There are usually a wide range of premiums available depending on such things as the coverage limits you decide upon. Of course the higher limits one selects, the higher you premium will be.</p><p>The most common legal actions taken against phlebotomy technicians center around negligence, the cases most often seen include:</p><ul><li>Severe damage to the nerves</li><li>A phlebotomist leaving a patient with a tourniquet still on causing swelling.</li><li>Permanent nerve damage due to the technician rushing a procedure causing hematoma.</li><li>Numbness from incorrect vein selection</li><li>Unsuccessful repeated attempts to draw blood resulting in hematoma.</li></ul><p>Even in cases where charges of negligence are proven to be wrong, it often takes years for such allegations to be cleared. New advances in both automation and instrument technology have lead to simplifying lab diagnostic tasks greatly improving the accuracy of test results. Unfortunately the high majority of errors have been found to exist in the pre-analytical portion of the procedure.  More than 90% of errors occur between the time that a physician orders blood work and the time that it is ready for testing. It is not possible to get accurate results if the specimen has been compromised in some way before it arrives at the lab for testing. Slightly more than half of all pre-analytical errors are due to a lack of either quantity or quality of the specimen. Other causes can be attributed to such things as:</p><ul><li>Inadequate training of the phlebotomy technician</li><li>Inappropriate container, collection devices and procedures</li><li>Errors in patient identification</li><li>Errors in patient preparation</li><li>Transport, storage, separation and handling issues.</li></ul><p>The right training and continual upgrading of skills is certainly an essential component in reducing pre-analytical errors. Budget constraints and staff shortages place an added level of stress on all phlebotomy technicians which can greatly distract from a technician’s focus on the job at hand. There is no question that winged collection sets are easier to maneuver, lightweight and provide a lower insertion angle making the procedure less difficult to complete and of course easier for instructors to teach.</p><p>While patients and phlebotomy technicians generally prefer butterfly use, many facility administrators limit their use. The reason is that winged collection sets can run as much as ten times the price of conventional safety needles. This poses a genuine concern for facilities that must confirm to budgetary constraints making their exclusive use virtually impossible in most health care settings.</p><p>Some administrators have implemented procedures for their use as opposed to withholding them altogether. One system that has proven successful is the setting up of a logbook that requires phlebotomists to checkout butterfly sets as they need them. Another method of controlling the use of butterfly sets is by making the technician physically go into a manager’s office to request a set each time they need one. Other facilities have found that the simple act of having to sign out butterfly sets has drastically reduced their usage. Obviously no phlebotomist should be allowed to compromise a blood draw in an effort to save money. They should however be aware of the tremendous cost and use butterfly sets only when they are called for. Phlebotomy programs must also be aware that budgets are an integral part of any health facility and instruct their students on when to use butterfly sets and when they are simply not necessary. No matter how meticulous a phlebotomy technician is or how terrific his/her communications skills are when dealing with clients, if a patient’s name is attached to the wrong specimens, disastrous and even fatal results can occur. In order to lessen these occurrences, the CLSI has issued new guidelines dealing with patient identification.</p><p>Patients that are able to provide their own personal information certainly ease this process. For those that are not capable of doing so due to a language barrier, age, cognitive impairment, etc. must be asked for proof of identification such as official ID cards, driver’s license, etc. or can have their identity verified by a family member or qualified caregiver. The fact that someone has responded to a name that has been called out does not qualify as patient identification. People of any age can be hard of hearing and many names may sound the same or can be misinterpreted due to mispronunciation. Once a patient has come forward to their name being called they should be asked to repeat their name as additional verification that you are in fact speaking to the right person. This will further ensure that no mistake has been made.</p><p>Additional new guidelines have also been recommended in the correct labeling of samples such as including the patient in the process if at all possible or at least performing the labeling process while the patient is present. Appropriate training sessions as well as assessing competency have also been recommended.</p><p>Advancements and improved techniques in both procedure as well as administration that cover all aspects of phlebotomy are put forth on a regular basis in order to protect technicians and increase the safety of patients. Successful completion of phlebotomy programs and subsequent <a
